Place all ingredients in a 5 quart (or larger) crock pot and cook on high for 6-8 hours.
When the pork reaches at least 165 degrees F. on a meat thermometer and the potatoes are easily pierced with a fork or knife, it's done.
Chop up the meat into bite-sized pieces.
Allow to cool and serve.
